[ ] indicates an optional item.
< > indicates a variable item.
... indicates repetition.  For example '<name> [, ...]' could be
    '<name>' '<name>,<name>' '<name>,<name>,<name>' etc.

This can be somewhat confusing since C uses [ ] for array subscripts,
< > for #include statements, and '...' for Variable Argument Lists.

However, it should be obvious from the examples when a [ ] is just a
[ ], etc.
